Agreement made this day of ___________________________ by  and between Cindy Wilson  And Mutts and Mates, LLC and __________________________________________________,  the Owner (s), and their dog (s) name _____________________________ and  ____________________________.

                          

1. OWNER REPRESENTATIONS

Owner represents that said pet(s) is in all respects healthy and has received all required and customary immunizations, and does not suffer from any illness, disability or condition which could affect said pets’ or other pets’ safety at Mutts & Mates, LLC. Events. 

 

2. ACKNOWLEGEMENT OF RISKS AND RESPONSIBILITIES

I/We the undersigned owner(s) recognize that there is an inherent risk of injury or illness in any environment associated with dogs in a group environment.  I (we) also recognize that such risks include, without limitation, injuries or   illness from fights, rough play, contagious   diseases and/or unwanted pregnancies.  Knowing these inherent risks, I/we warrant that I (we), the owner (s) of_______________________________ breed________________________,                will abide by all safety rules and instructions and hold Cindy Wilson, Mutts & Mates, LLC its agents, employees, volunteers, other members and event personnel harmless for said pet’s injury, death, or damage as the result of any incident including my/our or other’s negligence, I (we) further agree to hold Cindy Wilson, Mutts   & Mates, LLC, its agents, employees, volunteers, other members, and event personnel harmless and indemnify it against  all damage costs, fees and business losses resulting from any claim I (we) make or cause to be made  against   Cindy  Wilson,  Mutts  &  Mates, LLC,  and   its agents, employees, volunteers, other members and event personnel for which it,  its agents or  employees are not ultimately held to be legally responsible.

 

      3. OWNER LIABILITY

I (we), the undersigned, expressly agree to be held responsible for any damage or cost incurred by my pet(s), including but not limited to destruction of property, as well as any injury inflicted upon another dog as a result of a fight or injury inflicted to another person, or an employee or volunteer.

 

     4.  LIMITATIONS

I (we), the undersigned, understand that if my dog displays out-of-control or aggressive behavior, that for the safety and health of my/our dog(s) and others, I/we will be asked to leave the premises   or if needed, with my/our permission; a muzzle will be placed on my dog for the protection  of other people and dogs.

 

I HAVE READ AND UNDERSTAND AND ACCEPT the terms and conditions stated in this Agreement and acknowledge that this Agreement shall be effective and binding between all parties.
